If you have an insurance plan that covers out-of-network acupuncture and related services performed by a licensed acupuncturist we can provide you with a "super-bill" for you submit to your insurance provider to receive reimbursement after you have paid our fee(s). We’ll do our best to answer your questions about insurance coverage, however our office is unable to verify whether your health insurance plan has coverage for out-of-network acupuncture.
We cannot guarantee that our services will be covered or reimbursed by your plan. Insurance carriers may place restrictions on who you choose to be your practitioner and on the amount they will reimburse you for.
Please consult your insurance company or your benefits representative for information about your plan's coverage.
Your time slot is set aside for you. We require patients to provide 72 hours notice for cancellations. The full fee will be charged for appointments cancelled or rescheduled with less than 24-hours. If you have missed an appointment with less than 72 hours notice, you will be responsible for 50% of the office visit fee. Thank you for your understanding.
If you find that you are running late for your appointment, please give the office a call at 385-279-2410 to let us know. We will do our very best to accommodate you.
You may park anywhere in the parking lot. There is a handicapped ramp on the west side of the building and our office is located on the first floor in the southwest corner of the building.
We accept cash, personal checks, credit cards and HSA/FSA cards. Payment is expected at the time of your visit.
At Eastern Bench, we believe that the treatment begins from the moment you enter the office. We pride ourselves on providing the most healing experience in a calm, clean and beautiful environment. In addition to a hand-washing sink located in the bathroom and kitchen, we also have hand sanitizer in every room. Our linens are laundered daily and all reusable equipment, such as cups and herb dispensary equipment are used once and then washed in a high-temperature dishwasher.
We only use sterile, single-use, disposable needles, which are discarded into bio-hazardous waste containers. Prior to needle insertion, the skin is swabbed with alcohol. All Eastern Bench acupuncturists are certified in Clean Needle Technique by the National Commission for the Certification of Acupuncture and Oriental Medicine (NCCAOM).
